An Affidavit of Late Registration of Birth is a written statement under oath by a person in support of an application for delayed registration of birth with the Philippine Statistics Authority (PSA). The affidavit explains the circumstances of the person whose birth registration was delayed and why the person was delayed in registering his/her birth with the PSA. 


2. When do you need an Affidavit of Late Registration of Birth?

An Affidavit of Late Registration of Birth is needed if your birth was not registered with the PSA when you were born and, as a result, you do not have a birth certificate. The affidavit supports an application for delayed registration of birth to get the PSA to issue you an official birth certificate, which is an essential requirement for many aspects in life (i.e. enrollment in school, job application, passport application).

The affidavit is a requirement of the PSA in filing the application for delayed registration of birth.


3. What information do you need to create the Affidavit of Late Registration of Birth?

To create your Affidavit of Late Registration of Birth, you’ll need the following minimum information:

If the affiant is the person whose birth registration was delayed

  1. The name and details (i.e. nationality and address) of the affiant;
  2. Birthday and birthplace of the affiant;
  3. The relationship of the parents of the affiant (i.e. married or not married); and
  4. The reason for the delay in registering the birth.

If the affiant is not the person whose birth registration was delayed (i.e. the affiant is executing the affidavit on his/her child’s behalf)

  1. The name and details (i.e. nationality and address) of the person whose birth was not registered;
  2. Birthday and birthplace of the person whose birth was not registered;
  3. The relationship of the parents of the person whose birth was not registered (i.e. married or not married);
  4. The reason for the delay in registering the birth; and
  5. The affiant’s relationship to the person whose birth was not registered (i.e. mother).
