Affidavit of Dependent not being a Recipient of a Scholarship Grant

Say under oath that your dependent is not a recipient of any scholarship grant from either the government or non-governmental organizations.

1. What is an Affidavit of dependent not being a recipient of any scholarship grant?

An Affidavit of dependent not being a recipient of any scholarship grant is a written statement under oath by a person (the “Affiant”) stating that his/her dependent is neither a recipient nor beneficiary of any scholarship or educational grant from either the government or non-governmental organizations (e.g. foundations).


2. When do you need the Affidavit?

This Affidavit is normally required as part of the application requirements if you are applying with the government (e.g. GSIS, OWWA) for an educational grant or scholarship for your dependent.


3. What information do you need to create the Affidavit?

To create your Affidavit, you’ll need the following minimum information:

  • The name and details (e.g. nationality and address) of the Affiant;
  • The name and details (e.g. relationship to the Affiant) of the dependent; and
  • The purpose for which the Affidavit is being executed.

4. How much is the document?
