You will need this affidavit when you need to clarify that documents containing different versions of your name refer to you. For example, if there is a difference in the names contained in your birth certificate and other government-issued IDs, this affidavit can help clarify that you are the same person referred to in those documents.


3. What information do you need to create the Affidavit?

To create your Affidavit, you’ll need the following minimum information:

  1. Name and details (e.g. nationality and address) of the Affiant;
  2. Two documents where the name appears differently; and
  3. Purpose for which the Affidavit is being executed.
